Hypercalcemia with a normal PTH

Here is a coronal view (of the head, neck, chest) of a sestamibi scan in a 60 year old woman with hypercalcemia. Her parathyroid hormone level was normal. What's going on?

Here's some more delayed images, which I have posted after joe made guess 1 and 2 in the first comment.

March Meeting of the Cacoethes Cognitum

The most recent meeting of the Cacoethes Cognitum was quite productive. Here are a couple of highlights:

Joe revealed that he is a strong supporter of the use of neuticles for all neutered pets. After much disbelief, He finally hushed the conversation by asking "What else is a dog going to lick?"

Matt the opthamologist asked to the Matt the urologist if their was an epidemiological link between peyronie's disease and dupuytren's contractures. I thought that was a just a bad joke, but apparently there is.

Painful Hypopigmented Lesion Photo Quiz

This a 23 year old hispanic female who presented to Joe during a medical mission with these gradually worsening painful hypopigmented lesions. Since most of the medical equipment and testing supplies that we consider routine were largely unavailable, this is about all the information we have to go by. Maybe Joe can give some more details in the comments.

Joe and I have both proposed different diagnoses for this lady. Now by taking a shot at her diagnosis, you can swing the vote in my or his favor or even better, give us a new one to consider. So get cracking!
